Physical Evaluation/Medical History Form - must be filled out by all 9th and 11th grade athletes and all athletes who are new to Brenham ISD

The following forms must be completed by every Brenham High School athlete and their parent/guardian each year. All forms must be returned before the first day of scheduled practice. Any student who does not have all the forms turned in will not be allowed to attend practice. There will be no exceptions.

All Brenham Junior High School athletes and their parent/guardian must fill out the following two forms before the first day of scheduled practice. Any student who does not have both forms turned in will not be allowed to attend practice. There will be no exceptions.
